More evidence for the bug, as I have the achievement on Steam but not the actual penance.
KNOWN ISSUE: Penances Not Awarded After Disconnection/Crash, Despite Appearing in Steam Achievements
The problem is for this Penance might be different since I HADN’T disconnected from the mission.
And what to do when I have a problem with penance On target (1) which is consisted of completing other penances.

It was at this time that I complained to a friend that I was unstable due to a backend error, not realizing that I was no longer getting headgear due to crashing due to a backend error.

Issue Description:
Unable to complete “On Target(1)” Penance. (This took place before full release on the day of reintroduction for the repair mission type into the game during the pre-order beta). I thought this issue may have been already told or solved by now but I couldn’t find anything here so I report this myself. Just as I had completed the last 7/7 Veteran specific mission type penance, I had received notifaction at the ending screen, that I had completed the “On target(1)” penance, and promtly got the side screen notification for doing so along with my long saught after helmet. Just as I recieved these notification at the end of mission screen the servers promtly went offline, and the servers went onto unexpected maintanence for about 40-60 minutes, they then reteurned online. When I booted back up the game and hoped in, It showed on the penance screen, I had completed “On target(1)” yet there was no helmet in my inventory. Due to this issue I am unable to progress this Penance further to “on target(2)” and 3. I WOULD LIKE TO STATE: I did recieve penance completion for the “Vantage point (2)” Penance which was my last task to be able to aquire “On target(1)” and it’s rewards (The helmet). I DID, recieve “Vantage Point(2)'s” reward which was the “dutiful” green portrait frame for veteran.

Penance is “On target(1)”
It’s been a week and a half since I have first been unable to to complete the penance, It is difficult to say weather it is “On target” its self that i must complete or an individual penance that is part of the requirements to try to unlock this helmet again, (to CLARIFY, the “on target (1,2,3)” penances require multiple other penances to complete the “on target penance” I cant tell which penance I would have to complete by now in order to try and get the rewards again, do I have to complete one criteria penance for me to get “On target(1)” or do I have to do ALL of them again?
This is quiet confusing for me because the penance list says I have completed all prerequisite penances for “on target(1).” It may be of help that I have been able to progress further with other penances that would unlock “On target(2), and (3)” but the only thing that stays locked is the “quest line/penance line” of On target itself that stays locked.

Within this past week I have recompleted all of the requirments to get “On target(1)” and I am still unable to finish and unlock “On target(1)” and its rewards.

As you can see above, I have the missions completed and no helmet shown. But I do have that yellow spot all the time which won’t go away. Which is part of the bug that isn’t showing the helmet for me.

So i have encountered a Penance Bug, that does not seem to have any requirements and can just happen, and you have more or less no way to protect yourself from it.
I realised it when i was trying to get the Penance “Not even close”, because t is the last one i need for my Psyker. Well, turns out i already did it, at least to Steam, since i got an Achievement that told me i did it. Two days ago.
And no, the game still hasnt Registered that i have completed the Penance.
Now, i do not know if there is anything you can do anything against it, and probably not even all players will have this Issue. Maybe I’m the first one.
I’ll see if i can find out anything about this, maybe there is some special Requirement for that Glitch to pop up, or maybe Steam itself is bugged, wich is an entirely other Issue in itself. We’ll see.
I did a “Gone bowling” achievment, game told me its done, achievment is in Steam profile.
But server crashed and in game its undone again.
Dont remember exact error code, me and my party were raging about this crap. Something about disconnect.
I can do it again, but shoulkd know about it.

Penance straight up didn’t counted even tho I genuinely finished & made sure my teammates weren’t damaging the monstrosity almost at all, even if they did it was as minimum as possible
Might be that I didn’t even did enough to begin with, maybe it was 89% instead of 90% damage done, but honestly it does not feel like its the case
It also does not help that THERE IS NO TRACKER from previous attempts, to actually see that
Sharpshooter penance On Target (1)
i have completed all of the other penances required for it but i think it is permanently locked, as the requirement is to ear certain penances that i already have and cannot get a second time
i just want my dang helmet

The penance i have completed but the game is not counting as completed is On Target (1).
The official solution to penances not being awarded after a crash is to re-earn them, but i cant do this as i cannot earn the same penance more than once, and the game correctly tracks me finishing all of the prerequisite penances for On Target (1)
I have screenshots and the achievements (for penances that have them) to prove i earned all of them
I’ve merged this in to the existing post, so our developers will see it.

I have the same exact issue
Everyone else in the lobby is unlocking higher and higher ranked head gear and wearing goggle hats as if to show off, but I am the only one not enjoying the content.
Will I have to continue playing for the rest of my life without content?
I’ve reported this glitch over and over again in various places, but I haven’t received any response from the officials, nor have they announced anything.

I had the penance “Cliffhanger” completed on a mission that i was doing with 2 friends and the game crashed, it is showing on steam as completed but not ingame. And i don’t really want to do it again , so , i will apreciate if anything can be done here to get the penance completed as i did.

I was able to complete the Shocking Stuff penance in game and steam awarded me the achievement however once leaving the mission I had noticed I do not have the penance completed.

Update: I was able to get the penance in-game and was rewarded properly for it, however also worth noting that the number of kills needed between the in game description and steam achievement are different.
