Having worked in videogame QA testing, generally the issues aren’t with the QA team. They’re usually pretty good about pointing out stuff that doesn’t work or that doesn’t feel great.
The issue is that the programming team doesn’t have any time on their schedule for the next 7 sprints, one of the design managers is stubbornly refusing to acknowledge the problem due to office politics or personal pride, the project lead made a copy-paste error in their log halfway through the sprint and forgot about it for a month, and none of it’s critical to operations or anyone’s incentives/goals on their next review.
