I’m on the way to reaching 2K hours at VT2 and I’m still annoyed by the occasional:

Silent Hookrat that spawns too close to make any sound before catching you.

Instant Gutter Runner that somehow spawns as a guided missile that catches you during the first millisecond of its sound queue.
With the hook you can at least apply map knowledge and stay away from spawn locations (you have to know all of them or else you’re snatched without counter-play) but with the assassin the solution (gameplay wise) is even less practical.
There are a few different ways to fix it, the most obvious is making so the hook can’t spawn too close to a player and making the assassin unable to lunge for the first 1~2 seconds after the sound queue starts.
It’s not really a problem on quickplay but on twitch and/or deeds it sometimes feels like you’ve been robbed. I’ve made terms with it and just accept that sometimes (not very often, thankfully) the game just presses the ‘you lose’ button and ‘gettin’ gud’ isn’t an option.
It’s a testament to the game that I only see these two things as truly cheap (I don’t play CW, so I don’t know what’s what there), but damn, I feel like the possible fixes are within reasonable reach.
