Kerillian’s talent Piercing Shot has always been bugged, if it hits a teammate, the refund has some chance to not work properly. The bug is very common and for years I asked people not to walk in front of me which means i’m asking them to not play the game.
I will link a mod which fixes this bug, and a clip of an example.
I believe this may be intentional, to prevent teamkill-abuse.

Upon a closer look, in the clip at around 7 seconds, i think you might´ve hit the shoulder/arm rather than the head. Possibly didnt hit the bot either though i cant swear on that.
Red line is where i think the head-hitbox divides from the shoulder, impact spot appears to be on the wrong side .
no you can hear the headshot ping and ive tested this many many times, and experienced it on enemies which are sitting absolutely still like phased trolls,
