Malestrom missions really highlight how badly designed the Chaos Spawn’s grab is between it’s ridiculous hitbox and how vulnerable it makes you. Other monsters will still target you and hit you while he’s holding you. Flamers, bombers and snipers will still hit you and you can’t do anything about it.
That’s actually intended. While the Mutant has grabbed he, his body mass does not block oncoming attacks or shots from elsewhere. He is essentially to the Nurgle team what an Ogryn is to your team. You can cast your spells through it. Just the Mutant doesn’t block ranged guns, either.
He’s meant to disable you and hold you in place as you get attack from other sides as well. The throw at the end is just the finale.
Don’t get grabbed thinking you’re safe because you stand with the back to a wall. You’re not.
I didn’t say anything about Mutants, I know you are meant to be vulnerable when you get grabbed by one, but it also plows through enemies so you don’t get wailed on every time. I’m talking about the Chaos Spawn’s impossible to consistently dodge grab that lasts much longer and leaves you as vulnerable.

I’m pretty sure their grab works in the same way. The only reason you usually don’t get hit is because it’s swings knock surrounding enemies around.
No, you don’t get attacked by mobs while grabbed by a monster with the exception of specials and other bosses.
