Penances
Is there some hidden determination as to when an in progress mission counts towards a penance and when it doesn’t? Like it has to be less than a certain % in progress or something? This is a little frustrating since I don’t have any control over whether I start towards the beginning or not.

I see this is acknowledged. Does that mean this is a bug and a fix is coming to make all games count? Or … something else?

It means a member of support has seen your post, and marked it as “acknowledged.” This could mean the next hot fix directly addresses this. Or it could mean no one mentions it again for the next year. Who knows!
However, for your specific issues, it might help to provide some more specific examples, and maybe SS or small video. What penance didn’t track? How do you know it didn’t track? It could be a problem with specific penances over the entrie system.
For example, what constitutes “dodging” a Crusher overhead is very specific, so that Penance is a little annoying and doesn’t feel like it tracks correctly.
In this case, it was the “complete N mission” type penances. So, get the W and the counter should go up.
Ugh, this just happened again. This time I was pretty close to the start of the auric maelstrom mission. We worked hard for it and no credit towards any of my auric mission penances (Untouchable Operative and Storm Survivor). Did not die. 
And again. Super smooth Enclavum Baross auric damn. No deaths by anyone. Didn’t count at all towards Untouchable Operative.
