There were 2 disconnections from my friends side.
During the last portion my game froze on the cutscene and unfroze on the completion screen.

Replayed the mission a number of times after but the penance did not update. It’s listed as completed on my Steam profile, but not in game.
Me and 2 friends had completed the mission and both me and my friend completed the “praise the emperor” archivement needed to unlock the helmet. But as soon as we completed mission we got backend error and when we logged in we had neither the portrait or the helmet.

Ive got marked for death achivement on mission after that on this mission server crush is happened, ive got this achivement in steam? but in game its count as 4/4 and still uncomplete, and now uncomletable
In the same boat here, Servers crashed right at end of level and I got 2 achievements, they are now unable to be completed. Inquisitorial Recruit is stuck at 7/7 and Beat’em’up for skullbreaker is stuck at 6/7, have since completed the same mission and achievement did not go through.

I also completed the “not even close penance” and it’s unlocked on steam but not on the game due to server crash, guess that means I’ll never get the apparel part if it’s locked?

Yes, but the first post you are talking about refers to some penances that have nothing to do with mine, I have completed several and I do not receive the cosmetic in the inventory, directly nor does the penance appear completed when it is reflected 7 /7

Issue Description:
I completed the achievement “Vantage point” right before the server crash. Servers went down and I was booted out. When I’ve managed to log back in I have the achievement completed (7/7) but not completed (rewards etc). There is no achievement in steam for this Penance that I can see neither locked nor unlocked. I cannot re-do this mission requirement as the bar is already full and the “Criteria” met. I have since played all 7 mission types again and this has not resolved the issue nor changed the status of my Penances.
If you look at the one’s people seem to be experiencing issues with these do not appear to have “Steam achievements” associated with them (Vantage point, warp battery etc)
Steps to reproduce:
Presumably crash the servers at the exact moment you complete the penance. I am unsure what other steps could be taken.

same issue, completed the last penant need for “on the target (1)” which was “vantage point” it says 7/7 completed however it appears as not completed.
