Any mission in particular? A description of the location? Video or screenshots?

I spawned in and then immediately fell through the map and died and then left so no idea. The map was not playable.

This is likely to be Silo Cluster, with the player spawning immediately before the first medicae station. I have been looking through the forums to see if I could find anyone else reporting it. I will write my own report.
