Constant Server Error at end of match

Issue Description (Required):

Game frequently disconnects at the end of a match.
Sometimes it also happens in the lobby, before a match starts or when I alt-tab.

Unfortunately I’ve already tried this and some other fixes I found from similar posts indicating the same issue. Hasn’t change anything.
