i tested on room but never show icon .
and does it effect more than1 person same time?
I’m not sure about the icon. I’ve always used it, but I can confirm that it does have an effect.
As far as I know, the effect only applies to the first person, either yourself or an ally within coherency, who takes damage to health rather than toughness.
