Second time crashing when 1 mission from end of Chaos Wastes

The first time I crashed right after loading into the final level, the arena one, but didn’t get a crash report. This time I crashed on the level right before that while swinging my melee weapon. Why did my game crash and how can I fix it?

It appears your GPU is crashing. There are unfortunately multiple reasons this may be occurring and determining the cause is often a process of elimination.
