As the title suggests,
I have completed this penance twice now, once @ 19 minutes, with 17 minutes spent under 1 bar, and a 2nd time @ 16 minutes, with 13 minutes spent under 1 bar. I have yet to unlock the achievement, despite completing the conditions on two separate occasions. Has anyone else run into this issue?

I just had the same thing. Another Zealot did get it and I did not. I was under 1 wound before the other player and did not heal at all except some from Holy Revenant but I never went over 1 wound of health. They got the penance completion but I did not.
Edit: mission time 18 minutes. Started from lobby. No disconnects by anyone. No deaths by anyone. I went down once. The other zealot went down twice but healed at medicae station in between. So for all intents and purposes they spent less time under 1 wound than I.
I just want cool candle shoulder pads, why must life be so cruel! Thanks for the info.
Does time spent downed or waiting to be rescued count as full health?
I think being downed breaks it. I was helping someone on the Discord with it and it took 3 tries (all 3 were under 20 minutes) and the common denominator was that the zealot trying to get it was downed at least once in each of the failed runs. Another zealot did get the penance in one of the "failed"runs.
Out of curiosity if/when this gets fixed will we still have to redo the penance or will the attempt where we “succeeded but bugged out” count?
