Monsters can sometimes spawn in dead or not at all in Chaos Wastes, can happen on others maps as well.

I think it must be linked to the trouble the AI sometimes has with pathing. If that’s true, they’re already working on it.
This has hapenned to me only once in CW though, out of several runs. It’s quite noticable on cata Righteous Stand.
