I don’t know the specifics, but there is a missing collision box for one of the walls in the newly decorated/updated Excise Vault Spireside-13 mission that you can walk through. Walking through allows you to fall through the map.
Steps to Reproduce (Required):
1.) Play Excise Vault Spireside-13 (any difficulty, any class)
2.) Progress to the point right after the first med station but right before the event where you need to raise the bridge to progress. In the room where it is possible for you to drop down into a smaller room right before the event, walk against the wall depicted in the screenshots.
3.) Fall. And die.
