Completed the event. Met all requirements. The frame rewarded showed the prompt that I earned it, yet when I went to Cosmetics to change frame, the “Purge The Unclean” frame was not available. Even restarted the game and not sure how come it doesn’t show up.

Update: problem solved after another restart of game from title screen.
Item appeared at the bottom of the list of frames available under the generic “Portrait Frame” label
