I own Vermintide I, I have the body skins from owning it, however, I don’t have any of the hats.
I did play Vermintide I for a few hours, hat still not present.
No ideia how to reproduce the error.

I’m not sure since it’s a long time since I’ve used those.
But iirc, the suits will cover the head too. So you have 1 cosmetic that will over display over the head cosmetic

I’ve just tried that and yeah, it worked.
A bit disappointed since I can’t cosplay Urist with how that’s set up.
