A down-to-up attack from chaos warriors deals damage through gromril armor on iron breaker. The bug seems consistent from my experience and only occurs with one specific attack.

I believe this attack deals damage twice, so the first instance of damage is absorbed by Gromril and the second one goes to your HP. I also think that, a long while ago, patch notes said it was fixed, but I might be wrong.
