Been looking forward to finally getting some action with this great feat, but this issue just causes it to be unplayable in my opinion.
You are missing a verb in your title. I suggest “to disappear”. 
But yeah, I have the same problem. As soon as “Holy Revenant” is equipped, the “Until Death” Icon disappears. That way it is not possible to know, when Until Death is available again.
