Once - Rare (<10%) - Unusual (<25%) - Common (<50%) - Often (<75%) - Constant (100%)
Rare have never seen it happen before nor did the others in my team at the time

Bosses spawn when you walk through a trigger. Rarely these triggers can be placed very close to each other, and it can be very easy to accidentally walk through both

If you play with added Monstrosities from Auric missions, you can even have extra bosses during the Traitor Captain fight. It’s pretty neat.
