"There is Nothing but the Abyss" refuse to unlock for me

I got one penance remaining for Zealot and it’s the “There is Nothing but the Abyss”.
The one where you have to heal 25% by using the Holy Revenant talent.

I can’t seem to unlock it, no matter how many times I intentionally try to. It seems permanently locked on 24/25.
Fully suspect it to be bugged for me.

Steps to Reproduce (Required):

  1. Pick Zealot.
  2. Pick Horde-clearing melee weapon.
  3. Play a Heresy+ difficulty mission.
  4. Intentionally eat all the attacks from the first good enemy wave that spawn in a swarm.
  5. Murder everything during the short duration of the talent triggering.
  6. Heal up and be amazed as the penance didn’t unlock, once again.
