Has happened twice already since the last patch. Client crashes during a mission at random, with “Access violation (0x0000005)” error mentioned in the post-crash dialogue box (see attachment) - and no prompt to rejoin the mission is shown upon restarting it.

Not known, just happens randomly during missions.

Yes, but I haven’t tried disabling them
