Two times since the Outcast Engineer Update, while playing Hunger in the Dark solo as Victor WHC on legend, the chaos spawn has grabbed me and thrown me out of bounds into the unplayable area.
Both times happened in the tunnels before getting to the cart, near the dead end room with the chest, cart, shelves in it (usually has healing in the chest).
Both times I ran around trying to get back into play until I fell through the floor into nothingness and the game ended as if I died.
Playing on Xbox One X. All available DLC installed.
