After completing the last of the scanning section in Hab Dreyko, and escaping through the tunnel on to the iron grating (before the 3 bridges that get blown up).
There’s a cylindrical drop in the grating. If you fall down there, you fall too far down to be rescued by team mates, and just hang until you die.

Okay. Sure. Here goes:
Play Hab Dreyko at least 99 times to try and complete the Mission Improbable penance. And it is a penance.
As you’re about to finish this penance to complete your 100th success, while carrying some lower level players chancing their arm, suffer hugely overpowered knock back from a reaper that pushes you over the edge you weren’t really close to.
Fall, and watch your level 40 comrades die around you, mystified why they can’t pick you up.
