Looks like Warren 6-19, the room near the end when you need to do some auspex scanning.

Never seen this happen on Carnival 2, weird.

I know if you run fast enough with Knife Zealot, you can get stuck in the side passages during the Archivum Sicko Rats alarm section which was very funny to see happen.
