I have recently purchased the cosmetic bundle that includes the Commemorative Head Screws for my ogryn and noticed that immediately after equipping it for the first time, my character portrait in the bottom left corner now has a permanently glitched visual. Equipping other head cosmetics does not fix the issue. On a whim I decided to use my empty character slot to make another ogryn character and after equipping the same cosmetic on the new character he received the bugged icon as well.
By no means the most game-breaking bug, but an odd one nonetheless.
From doing a little more testing, it seems the actual issue is the body cosmetic, not the head cosmetic. Swapping off of the Sump King’s Leathers to another torso piece fixed the icon, and swapping back to it broke it again. I’ve attached a couple more pictures of what the icon looks like with the default prisoner garb and the Commemorative Head Screws cosmetic, and then what the icon looks like with the Sump King’s Leathers and the default head.
