This is on the fuel refinery map. When we got into the elevator at the start of the map we hit the button to get the elevator moving. Then a mutant came along and grabbed me out of the moving elevator, slammed me down a few times and threw me. I somehow ended up back inside the elevator, however about halfway up the shaft my camera started to shake wildly before I fell through the elevator to my death. Picture taken after impact.
