As you can see in the screenshot this little staircase lacks a railing on the side. I didn’t notice this and I side-dodged off the edge and ended up hanging there. When my teammates picked me up instead of standing on the platform next to the ledge I was instead teleported up through the ceiling and into a group of enemies on the level above.
