In Ascension Riser 31, if you go down at the point where it sends you to the rescue point in the train tracks (about halfway through the mission, right after climbing the collapsed walkway into a room and then dropping through a hole in the floor to progress) you can be teleported beneath the floor of the rescue point, rendering you completely unable to be revived.

Steps to Reproduce (Required):

  1. Okay Ascension Riser 31
  2. Go down in the room after moving up the collapsed walkway and die
  3. Get teleported into the ground and become unreachable.
