The Iron Helm MKIV Thunder Hammer seems to have a problem with its second charged up swing going from left to right (At least that’s the one I notice most). It will charge all the way up, but then when released it won’t swing the attack, it just cancels and returns the character to neutral stance. It has happened both with and without the special activation, and I believe it happens more when sprinting/moving forward rather than side to side movements. I am currently using shroudfield and notice it happening often while sprinting forward. I have not noticed any problems with regular swings.

I have tested this and found that when sprinting forward (Or simply holding the sprint key down while attacking) 100% of the time the attack will cancel both the heavy attack and the light attack that go from left to right (Second swing) I was unable to replicate it, but there was a hiccup for the third overhead attack cancelling out while sprinting forward as well, I’m unsure how it occured. I have not tested this with any ability other than Shroudfield at current.
It is like that from release xD It was such a huge gripe of mine since I was playing a lot of TH Zealot back then
Tested it more today. I figured out the exact issues- While sprinting, regardless of whether using ‘Unfaltering’ talent or not, or whether you begin the attack during a sprint, or begin to sprint during the attack.. The second attack in the chain will reset if not Fully Charged during the Heavy Attack swing. This also occurs for the second Light Attack while sprinting forward, it will do the first attack, then stutters on the second, forcing you to repeat the first attack. The Third and final Overhead Heavy Attack will do the same exact thing as the second Heavy Attack. If you do not fully charge the Heavy Attack, it will simply cancel out the entire attack and return to neutral stance/first attack sequence.
I never had this problem before, and I’ve used this hammer since the day it was made available in game. I took a long break and came back to find it’s like this. It’s frustrating without a doubt because it’s one of my favorite weapons 
It absolutely was and is still happening, specifically with TH, if you try to sprint into charged attacking, while having both dodge and sprint bind on shift
